Can Vitamin B3 Help Protect Your Vision from Glaucoma?

Glaucoma is one of the leading causes of irreversible blindness worldwide. For years, the only proven treatment has been lowering eye pressure — through drops, laser, or surgery. But what if a common vitamin could offer additional protection for the delicate nerve cells in your eye?
A new clinical trial published in the *British Journal of Ophthalmology* suggests that **nicotinamide — a form of vitamin B3** — may help improve retinal function in people with a type of glaucoma called normal-tension glaucoma.
## What the Study Found
Researchers in South Korea enrolled 53 patients with normal-tension glaucoma in a carefully designed clinical trial. Each patient received either nicotinamide supplements or a placebo for 12 weeks, then switched to the other treatment.
The dosage started at **1 gram per day for six weeks**, then increased to **2 grams per day for another six weeks**.
The results were encouraging:
- Patients taking nicotinamide showed **significant improvement in retinal nerve cell function**, measured by specialized eye tests called electroretinography
- The improvement was seen in two key measurements (PhNR and B-wave amplitudes) that reflect how well the retinal ganglion cells — the very cells damaged by glaucoma — are working
- Nearly **29% of patients** on nicotinamide showed meaningful improvement, compared to 19% on placebo
## Why Nicotinamide?
Nicotinamide (also called niacinamide) is a form of vitamin B3 found in many foods, including meat, fish, beans, and fortified cereals. It plays a crucial role in **cellular energy production** — helping your cells generate the fuel they need to survive and function.
In glaucoma, retinal ganglion cells gradually lose their ability to produce energy efficiently. Scientists believe that boosting NAD+ levels (a molecule nicotinamide helps produce) could give these struggling cells the energy support they need to survive longer.
Earlier laboratory studies in mice showed that nicotinamide prevented glaucoma damage in **93% of eyes** at the highest doses tested — a remarkable result that inspired these human trials.
## What the Experts Say
The **American Academy of Ophthalmology** and the **American Glaucoma Society** have issued a joint position statement on nicotinamide:
- Nicotinamide shows **promising early results** but is **not yet approved** as a glaucoma treatment
- If used at doses below 3 grams per day, patients should work with their primary care doctor and have **periodic liver function tests**
- Doses of 3 grams or more per day are **not recommended** outside of clinical trials
- Patients should **never substitute niacin** (a different form of vitamin B3) for nicotinamide, as niacin carries known liver toxicity risks
- Any side effects should be reported to a healthcare provider immediately
## Important Limitations
While the results are exciting, there are important caveats:
- **The study was relatively small** (53 patients) and short (12 weeks per treatment phase)
- **Visual field tests did not show improvement** in this timeframe — glaucoma progresses slowly, so longer studies are needed to see if the retinal function improvements translate into preserved vision
- **Two cases of liver injury** have been documented across all nicotinamide trials to date, so medical supervision is essential
- This is **not a replacement** for your current glaucoma treatment — eye pressure management remains the foundation of care
## What This Means for Patients in Dushanbe
Glaucoma affects many people in Tajikistan, and access to the latest treatment options is important. While nicotinamide supplements are widely available, **please do not start taking them for glaucoma without consulting your eye doctor first**.
The research is promising, but we are still in the early stages. What you can do right now:
- **Keep your regular eye exams** — early detection is still the most powerful tool against glaucoma
- **Use your prescribed eye drops consistently** — pressure control remains essential
- **Ask your doctor** about whether nicotinamide might be appropriate for your situation
- **Eat a balanced diet** rich in B vitamins, leafy greens, and omega-3 fatty acids to support overall eye health
